**To receive the before OR after school rate, a consumer must attend only before school all week or after school all week (Mon - Fri).

1. If 2 consumers attend from the same household, the family will receive 10% off the highest individual fee and will pay the full fee for the other consumer. The family must be paying the fees for both consumers.

2. Services may be denied to families with past due accounts.

3. A $30.00 charge will be added to your account for returned checks.

4. If a consumer is absent on an otherwise scheduled day, full charges are assessed. This applies to all consumers-private pay and HCBS waiver funded.

a. Example 1: BASICS consumer who is scheduled to attend 5 days a week and private pays, is absent for 3 days -the weekly fee remains $65.

b. Example 2 : BASICS consumer who is scheduled to attend 5 days a week - uses Respite and/or SCL all 5 days – absent for 3 days - absence fee is $39 ($65/5=$13 x 3 days = $39)

5. Consumers attending the program year-round are eligible to take 5 days vacation without charge. The 5 days may be taken after the consumer has been enrolled for 6 months. This does not apply to consumers who attend only during the school year or consumers who attend June-August.

6. A late fee of $10.00 will be charged for every 15 minutes that a consumer is left after the center closes (6:00 pm). This means that a $10.00 charge is incurred after the center closes with an additional $10.00 fee after the first 15 minutes have elasped, and every 15 minutes thereafter until the consumer has been picked up.

7. Drop-In care must be arranged 24 hours in advance and all handbook specifications met
