These forms can be filled out using Adobe Acrobat Reader. Just click on the form and Reader will open, then click on the icon that looks like an (I )and click in the form. You can either tab through the form or click in the space where you need to type. After you are done filling out the form, at the top you can click on one of the buttons to print the form off or e-mail it directly to the correct department.
